Health center services are offered to the entire NCWC campus community.
The Health center offers a comprehensive approach to health and well
being through education, consultation, and referrals. The Health center
is open Monday through Friday when classes are in session from 9am to 4pm. The walk-in clinic or sick clinic is
open from 9am to 12pm. The Health Center is contracted through Dr Dan
Crocker's office, at Eastern North Carolina Medical Group (ENCMG). A
Physician Assistant is available in the campus Health Center three days
a week - Tuesday, Thursday, and Friday from 12:30pm to 2:30pm. In addition a
Registered Nurse is available for basic medical care and referrals. The
Health Center operates as an extension of the Physician's office. In
other words the students/staff/faculty need to come to the Health
Center with insurance cards and co-pays, if applicable, for medical
care. Call 911 for emergency assistance. If you have any questions
or need to speak to the nurse call (252) 985-5186. A. Immunization Required The state of North Carolina requires that all students entering college must provide a record of immunization. The record must document 3 childhood DPT (Diphthia, Pertussis, and Tetanus) injections, tetanus booster given within the last ten years, and 2 MMR (measles, mumps, rubella) vaccines. The Health Center does not require a physical examination for admission to the college. [This does not affect established requirements for student athletes.] A TB skin test is required for international students. Appropriate follow up documentation must be provided to the Health Center for all positive TB skin test. North Carolina law dictates that all students provide proof of completed medical requirements. If the required documentation is not provided, the student will not be allowed to attend NCWC. Any questions about medical records requirements should be directed to the Health Center at (252) 985-5186. The health form can be downloaded here, and can be mailed or faxed to 252-985-5499. B. Meningococcal Vaccine NCWC Health Center will have available to all students' information regarding the meningococcal vaccine. The information will cover the disease, benefits of the vaccine, risks, and where to obtain the vaccine. All students' vaccine information is confidential. Go to section titled "Updates" for more information on the meningococcal. C. Student Insurance All day students taking 12 hours or more are required to have health insurance. This can be your own or the college's health insurance policy. If you choose to use your own health insurance, it must have as a provider Eastern North Carolina Medical Group (Doctor Dan Crocker). You must submit a waiver form with a front and back copy of your insurance card to the health center by August 1 for the fall semester and January 3 for the Spring semester. This must be done on an annual basis. One of the great benefits of attending NCWC is that we provide accident coverage at no cost to all full time day students. The college also offers sickness insurance for a one time rate. This insurance coverage includes:
However, you will be placed on the college's insurance unless you fill out a waiver form completely and attach a front and back copy of your insurance card to the waiver and return them to the Health Center by the appropriate deadline. The annual waiver card and brochure will be sent to you during the summer. When you need medical services, you must take a claim form with you which can be downloaded below. If you need a prescription filled, you can submit a receipt and Prescription Claim Form to Academic Health plans for reimbursements. To find a participating pharmacy in your area, call WellDyne Rx at 1-888-479-2000 or go to the www.welldynerx.com. D.
